不必要的難與複雜

……在現代的資本主義社會生活中，大概沒有其它的職業比醫藥業更享有如此祟高的「專業」光環與令人敬畏的地位了，也大概沒有什麼職業的資訊比得上醫藥業的泛濫了！每家報紙都有保健衛生版，專業的保健衛生雜誌充斥坊面，保健衛生的叢書擺滿大書店的一整排，網路天天流傳關於衛生保健的知識，它的泛濫已到了真偽難辨、目不暇給的地步了！

然而，人們並未因此而能提昇衛生保健的DIY能力，主宰健康的權力依然操在醫療制度與醫療「專業」工作者－－醫師－－的手裡；幾乎在所有的消費行為之中，人們都保有「買家」的倨傲姿態與心理，唯獨遇上了醫師，人們就「自然」矮了半截；看著他／她書寫著潦草而無文理解的英文診斷記錄，專注而謙遜地回答他／她的問診，想要探詢卻不敢啟口，心有懷疑卻不敢爭論，這是在達官權貴與財團董事長前也沒有的溫馴，即使在這般醫療市場競爭的時代，作為消費者的我們依自不可能表現出「貨比三家」的拿蹺態度而對醫師稍事造次。

這究竟是為什麼呢？

不知有沒有人對於此種不可能挑戰的醫／病權力宰制關係感到好奇呢？

這道問題的嚴謹回答，可能需要成篇累牘的論文，不過，以下名作家楊照在＜聯合報＞副刊的短文《不必要的難與複雜》，或許能給人們帶來些許的啟示吧……

不必要的難與複雜，是創造利益的不二法門．．．．
在街上看到＜衛生署＞做的公車廣告，教人家用藥該小心的事。廣告上用大大的字標出五個你應該要問醫生的問題。第一個問題是：這是什麼藥？

我突然有忍不住笑出來的衝動。不是笑＜衛生署＞的廣告，這廣告沒什麼錯沒什麼好笑的，而是我腦海中浮出的景象讓我想笑。

「醫生，這是什麼藥？」病人問。如果他遇見的是最老實的醫生，或者是最狡猾的醫生，那他得到的回答可能是：「Chlorpheniramine。」「什麼？喔，那這個又是什麼藥？」「Clemastine。」病人如果堅持再問下去，他會得到更多這類的字，acetaminophen， pheny- lephrine， noscapine， ibuprofen, antihistamine， petrolatum-based ointment， 3C protease inhibitor．．．最後醫生會說：「這個藥我也不會唸，我拼出來你自己唸吧 ——r-u-p-r-i-n-t-r-i-v-i-r——」

好難唸吧。好深奧的藥物啊，如果我是病人，我大概會說：「對不起，算了算了，我不需要知道這是什麼藥了。拜託告訴我這藥治什麼、怎麼吃、吃多久就好了。」

如果我是病人，聽到說我要吃的藥那麼艱難複雜，搞不好會驚慌失措，以為自己得了什麼嚴重、了不起的病哩。

其實上面那些藥，都是治感冒時常用的。只是有的舊、有的新，根本上都處理我們日常生活最普通不過的病徵。跟什麼重症、疑難雜症統統都扯不上關係。

普通的藥，名字怎麼如此複雜難懂更難記呢？大學時代雜混在一群藥學系學生間上「藥理學」時，我就問過這個問題。當時我從老師和同學那裡得到的答案是：「這些名字都有化學上字根字尾的道理，所以非這樣不可。」「就像生物都有精確學名一樣，藥名必須長而複雜，才能跟日常用語分開，自成準確的系統。」

我當時沒有懷疑，也沒多抱怨。正因為藥名那麼難，念「藥理學」其實得靠一點死背的功夫，於是歷史系出身的我才能混在藥學系同學間，還拿到不錯的成績。

後來去了美國，在一個PARTY中巧遇一位對文學、對歷史都非常有興趣的年輕外科醫生。我們從康拉德的小說開始聊起，聊一聊聊到了醫藥的領域，我告訴他我的短暫的藥學經驗，以及當年苦記藥名的痛苦，他突然露出狡獪的笑容，一直搖頭說：「不是這樣，我告訴你，真的不是這樣。」

什麼不是這樣？「很多藥名是研究室、藥廠取的。」他說，「我待過這些藥廠，他們故意取難記的藥名，這樣一般人永遠搞不清藥的成分，只會記得藥廠要宣傳的成藥的名字。難與複雜，不必要的難與複雜，是創造利益的不二法門呀！」

不必要的難與複雜，是創造利益的不二法門。我記得了。也希望大家也都記得。

註：或許還應再加上兩句：不必要的難與複雜，是創造利益的不二法門，也是創造意識型態迷思的不二法門，更是創造權力宰制的不二法門。
